If you feel that you are suffering from a complaint that could benefit from physiotherapy, you are now able to make a direct referral to the department at Darent Valley Hospital. This offers patients greater choice and faster access to hospital services without the need to visit the GP.

Healthshare Physiotherapy for NHS patients

You can refer yourself to Healthshare for physiotherapy if you:

  • Have a muscle or joint related issue/condition
  • Are registered with a GP within the areas we operate
  • Are over 18 years old
  • Are not under the care of a hospital consultant for the same problem
  • Live in one of our Hull, London, Dartford, Gravesend or Swanley catchment areas.

Through confidential, one to one sessions we can help you feel motivated and overcome barriers. We'll help you introduce some lifestyle changes and also signpost you to further support you may need like mental health, debt or housing.

 

Kent County Council Care Needs Assessment

The care needs assessment looks at how your needs impact on your wellbeing and the outcomes you would like to achieve in your daily life. We will assess your care and support needs with you, and decide if they are at the level where you need support.

Your needs could be eligible if you are not able to do a combination of certain things that significantly affect your wellbeing. These may include:

  • washing yourself
  • getting dressed
  • going to work, college or volunteering
  • keeping your home safe for you to live in.

You can either:

  • complete a self assessment
  • request a care needs assessment
  • ask a professional (for example your GP) to refer you.

To book your pregnancy care you do NOT need to visit your GP for a referral

Please click on the above link to book your antenatal care and delivery with Darent Valley Hospital. After you submit this form a notice will appear on your screen confirming it has been submitted.  You will also recieve a copy of your completed form to the email address you have provided.

Your maternity appointments will be sent to you via email, unless you choose not to use this option. If you have not chosen to receive your appointment via email, it will arrive via post. We aim to provide you with the earliest available appointment in your area and provide a scan date from between 12 weeks to 13 weeks and 6 days of pregnancy.
